All cats/kittens over 4 months of age have received the following:
     Examined by a Veterinarian
     FeLV/FIV tested
     Feline Distemper Vaccination (The FVRCP vaccine)
     Rabies Vaccination
     Flea/Tick Treatment
     Ear Mite Treatment
     Dewormed
     Microchip (if in the City of Columbus) ($25 chip if outside the city limits of Columbus)
     Spayed or Neutered
